PRIVACY & CLIENT RIGHTS

Your Right to a Good Faith Estimate

You deserve clear information about the cost of your care before you begin.

You have the right to receive an estimate of your expected health care costs.

If you don’t have health insurance, or you aren’t using insurance to pay for your care, federal law generally entitles you to a written Good Faith Estimate when you request one or schedule services at least three business days in advance.

What your estimate includes

Your estimate explains the expected charges for your care, including the services, frequency, and period of treatment reasonably anticipated at the time. The estimate is personalized to your planned care. This webpage is a notice of your rights, rather than your individual estimate.

How to request an estimate

You can ask for an estimate before deciding whether to schedule. Email eunice@eunicelimlmft.com or call (909) 212-1110 and let me know you’d like a Good Faith Estimate. You don’t need to share detailed clinical information in your initial request.

  • If you request an estimate before scheduling, it generally must be provided within three business days.
  • If you schedule three to nine business days ahead, it generally must be provided within one business day after scheduling.
  • If you schedule ten or more business days ahead, it generally must be provided within three business days after scheduling.

I typically provide your Good Faith Estimate electronically so you can save or print a copy. If you prefer a paper copy, please let me know. If your expected services or charges change, an updated estimate may be needed. An estimate does not require you to begin or continue treatment.

If your bill is higher than expected

If a provider’s bill is at least $400 more than that provider’s Good Faith Estimate, you may be eligible for the federal patient-provider dispute resolution process. You generally must begin the dispute within 120 calendar days of the date on the initial bill. Keep a copy of your estimate and your bills.

For details about your rights, estimates, and the dispute process, visit CMS.gov/nosurprises.
